Resumen
Objetivo

Describir las infecciones invasivas por Streptococcus pyogenes (iGAS) que requirieron ingreso en unidades de cuidados intensivos (UCI) en España y comparar sus características epidemiológicas, clínicas, microbiológicas y de evolución antes y después de la pandemia de COVID-19.

Diseño

Estudio observacional retrospectivo multicéntrico.

Ámbito

21 UCI españolas.

Pacientes

Adultos ingresados en la UCI con infección iGAS confirmada entre 2018 y 2023.

Intervenciones

Se analizaron los datos clínicos, epidemiológicos y microbiológicos registrados.

Variables de interés principales

Incidencia anual de iGAS, foco de infección, gravedad clínica, necesidad de soporte respiratorio avanzado, estancia en la UCI, mortalidad y perfil de sensibilidad antimicrobiana.

Resultados

Se incluyeron 195 pacientes (59% varones, mediana de edad 55años). Las infecciones de piel y partes blandas fueron el foco más frecuente (53%), seguidas de la neumonía (39%). La incidencia anual de iGAS descendió al comienzo de la pandemia, alcanzó su valor más bajo en 2021 y posteriormente aumentó en 2022-2023, con una incidencia aproximadamente dos veces superior (RR=1,96; IC95%:1,47-2,62). Este incremento se debió principalmente a la neumonía, que representó el 44,5% de los casos, frente al 28,4% en el periodo prepandémico (p<0,001). Los pacientes del periodo pospandémico presentaron mayores tasas de SDRA, mayor necesidad de ventilación mecánica prolongada y de ECMO, así como estancias más largas en la UCI, sin diferencias significativas en la mortalidad (≈20%).

Conclusiones

Tras la pandemia de COVID-19, los ingresos en la UCI por infecciones iGAS aumentaron significativamente en España, principalmente a expensas de un mayor compromiso respiratorio. Estos resultados reflejan cambios relevantes en la epidemiología de S.pyogenes y subrayan la importancia de una vigilancia continua tras pandemias virales.

Abstract
Objective

To describe invasive Streptococcus pyogenes infections (iGAS) requiring intensive care unit (ICU) admission in Spain and to compare their epidemiological, clinical, microbiological, and outcome characteristics before and after the COVID-19 pandemic.

Design

Multicenter retrospective observational study.

Setting

21 Spanish ICU.

Patients

Adult patients admitted to the ICU with confirmed iGAS infection between 2018-2023.

Interventions

Clinical, epidemiological, and microbiological data were analyzed.

Main variables of interest

Annual incidence of iGAS, source of infection, clinical severity, need for advanced respiratory support, ICU length of stay, mortality, and antimicrobial susceptibility patterns.

Results

A total of 195 patients were included (59% male, median age 55years). SSTI were the most frequent source (53%), followed by pneumonia (39%). The annual incidence of iGAS declined at the beginning of the pandemic, reached its lowest level in 2021, and subsequently increased in 2022-2023, with an approximately twofold higher incidence (RR=1.96; 95%CI: 1.47-2.62). This increase was mainly driven by pneumonia, which accounted for 44.5% of post-pandemic cases compared with 28.4% in the pre-pandemic period (P<.001). Post-pandemic patients showed higher rates of ARDS, greater need for prolonged mechanical ventilation and ECMO, and longer ICU stays, while mortality remained stable (≈20%).

Conclusions

Following the COVID-19 pandemic, ICU admissions for iGAS infections increased significantly in Spain, largely due to a rise in respiratory involvement. These findings highlight substantial shifts in S.pyogenes epidemiology and emphasize the need for ongoing surveillance after viral pandemics.
