History of the coloids
2023, Acta Colombiana de Cuidado IntensivoLa sepsis es una de las causas fundamentales de atención hospitalaria, tanto a nivel de Urgencias, como de las unidades de Cuidados Intensivos, así como de las plantas de hospitalización. El reconocimiento de este síndrome, así como el inicio de un tratamiento temprano, ha demostrado disminuir la mortalidad de nuestros pacientes. Dentro del tratamiento de la sepsis se encuentra, además de los antibióticos, la reanimación agresiva con fluidos, siendo los más frecuentemente empleados los cristaloides. Sin embargo, existen otro tipo de fluidos, como los coloides, más en concreto la albúmina, que se podrían considerar a administrar en el proceso de resucitación del paciente séptico cuando estos requieran grandes cantidades de cristaloides durante el mismo. En la presente revisión se realizó una búsqueda sistemática sobre artículos publicados en inglés y español, sin fecha límite, dentro de las bases de datos de Pubmed, Medline, Latindex y Google Scholar, y aquellos que incluían las palabras claves «colloids», «history of colloids», «sepsis», «fluids», «resuscitation fluids» y estudios relacionados. Se consultaron, además, temas puntuales de acuerdo con las referencias bibliográficas presentadas. Este trabajo ha permitido construir una descripción narrativa de la historia mundial del origen de los coloides, sus luces y sus sombras, así como la situación actual en todo lo que refiere a este tipo de fluido de reanimación.
Sepsis is one of the fundamental causes of hospital care, both at the emergency level, as well as in intensive care units as well as hospitalization facilities. The recognition of this syndrome as well as the beginning of an early treatment, has shown to decrease the mortality of our patients. Within the treatment of sepsis, in addition to antibiotics, aggressive resuscitation with fluids, the most frequently used are crystalloids. However, there are other types of fluids, such as colloids, more specifically albumin, which could be considered to be administered in the resuscitation process of the septic patient when they require large amounts of crystalloids during it. In the present review, a systematic search was carried out on articles published in English and Spanish, without a deadline, within the Pubmed, Medline, Latindex and Google Scholar databases and those that included the keywords “colloids”, “history of colloids”, “sepsis”, “fluids”, “resuscitation fluids” and related studies. In addition, specific topics were consulted according to the bibliographical references presented. This work has allowed us to build a narrative description of the world history of the origin of colloids, their lights and shadows, as well as the current situation in everything that refers to this type of resuscitation fluid.
Ethics research in critically ill patients
2018, Medicina IntensivaLa investigación en el enfermo crítico es una obligación ética. Los conflictos éticos de la investigación en medicina intensiva provienen de la vulnerabilidad del enfermo, que en ocasiones ha perdido o visto reducida su capacidad de decidir, perdiendo así su autonomía. Debemos por tanto aunar esfuerzos para que ni los tratamientos, como la sedación o la ventilación mecánica, ni la propia enfermedad cercenen el derecho a la libertad individual de los participantes en la investigación, favoreciendo las condiciones en que hemos de recabar el consentimiento informado. La fragilidad, entendida como el compromiso de la capacidad para tolerar efectos adversos derivados de la investigación, ha de ser tenida en cuenta en la selección de los participantes. Esta ha de ser pertinente, imposible de aplicar en pacientes menos graves y ofrecer a priori unos beneficios potenciales que superen unos riesgos que han de ser conocidos y asumibles en función de una ética de la responsabilidad.
Research in critical care patients is an ethical obligation. The ethical conflicts of intensive care research arise from patient vulnerability, since during ICU admission these individuals sometimes lose all or part of their decision making capacity and autonomy. We therefore must dedicate effort to ensure that neither treatment (sedation or mechanical ventilation) nor the disease itself can affect the right to individual freedom of the participants in research, improving the conditions under which informed consent must be obtained. Fragility, understood as a decrease in the capacity to tolerate adverse effects derived from research must be taken into account in selecting the participants. Research should be relevant, not possible to carry out in non-critical patients, and a priori should offer potential benefits that outweigh the risks that must be known and assumable, based on principles of responsibility.
