In contrast, although D-dimer is a sensitive test, it may not be beneficial to evaluate the severity of sepsis-associated DIC since the level does not increase with the severity due to fibrinolysis suppression in sepsis (47). The increased release of PAI-1 and subsequent fibrinolysis suppression is the hallmark of sepsis-associated coagulopathy, but limitations include standardization, and represents a reason why D-dimer was eliminated from SIC criteria (5).
